Premium probably just depends on what the girl or her agent decides her rate is. What the rate is doesn't necessarily reflect service but it overlaps as better service attracts interest/demand and higher prices.
Japanese girls are always premium.
Case by case with Chinese girls (popular girls usually premium)
Also case by case with euros, both on their current roster are premium priced. Sapphire is more available than Natalia (who hasn't been rostered for a while).

It's hard to say whether it reflects quality of service because some girls decide to charge lower as they prefer more volume over less clients. I'd say this particular girl was probably working at another shop and charged out at a rate that's lower than the premium vixens price (which is why she is classic).
If we use BT as an example, I often received better service from girls that weren't at their top tier. They had one top tier girl that blew everyone else out of the water and drew me in thinking that the other top tier girls were similar. Some came close, none matched.
