Catching an STI that way is pretty unlikely. You're more likely to get hurt in a car crash on the way there than to catch anything serious from one session.
That said--nothing's zero risk. Some stuff like herpes or throat infections can happen. If you want to be safer, wear a seatbelt while you're in the car, and use a dental dam while you're going down (or just don't do it).

Harry's answer is so on point, it's hard to add anything.
Like he said, it's herpes if there's an active infection that's the main one, but the chances are quite small, nearer the 1% mark. The other yuckers are even lower.
An uncovered BJ though? If she has an active clap infection going in? 1 in 200. Folks better hope you're not her 200th as its mostly asymptomatic in the throat and I don't know what the check rate is for oral rather than genital STI screenings.
