I won't speak to other's preferences, but for me if its listed as included in the service booked then you can assume you have consent for it

... this leads to my biggest pet-peeve, which is when clients book a gfe and then start introducing extras without checking first. I know clients want to get the best value out of their bookings (and everyone has feelings about extras.) I get that! But to me, this is the sneaky version of haggling - what is being (wordlessly) asked is "Will you do a PSE for your GFE rate?" And yeah, I'm going to say no, because it's disrespectful and never fails to put an edge on the booking.

I put careful thought into my rates - I don't like to overcharge, but I won't undercharge either.

Extras are extra for good reasons, we just don't spell them out because it detracts from the fantasy (And no, its often not about disliking them, there's just unsexy practicalities to consider)

Other punters have covered this off. I would describe it as just being 'present' with undivided attention on the client for the duration of the booking time. That has been my experience in the significant majority of bookings. However...

I have had knocks on the door, conversations through the wall, get up and leave the room, insipid post sex massage with frequent pauses to answer texts, booking cut short because the next mug was there for a booking.
