I suspect it's a common problem but of course most of us don't like to be too open about our failings...
Some things that used to cause me some problems:
1. Holding off cumming for too long to "get my money's worth".
2. Getting too concerned about whether I'm pleasing her rather than concentrating on pleasing myself. For me, this can be an issue when he girl seems to be enjoying herself and I focus too much on that and wind up going for too long. Same effect as case 1 above.
3. Girls who get noisy - either one of those annoyingly fake porn-star acts or a some moaning that I mistake for her being in pain.
For me, the key is to keep in mind that I'm there to enjoy myself. Of course, I'm not advocating treating a girl badly, and if she has a good time too, that's great. But, especially with a new girl, from the moment she opens the door I keep reminding myself how much I'm going to enjoy cumming inside her (with appropriate protection of course....).
I find repeats much less stressful as I know what to expect and what about her turns me on, so if I find someone I particularly like, I go back...

^This. Give and take when you're not paying for it. When you are, many providers will be satisfied if you've enjoyed yourself, not if you've spent that time trying to satisfy them.

It's probably a mental thing for some of the reasons discussed above.
I can't stand girls who get noisy. The worst is when working girls do the fake moaning. They think we can't tell but most guys can, we just don't say anything lol. I think I would prefer them to not moan at all than to fake moan to be honest.

What EzyRider said, plus I'd add that I had quite a few punts before I first popped with a WG and since then it gotten a lot easier. For me I'm sure it's a mental relaxation thing although I suspect that the fact I had gone through a long dry spell before I started punting meant that my body had kinda 'forgotten' how to cum with stimulation other than wanking.
